To speed up your registration and evaluation process when you arrive at RICBT, we are pleased to make some of our initial paperwork available here. Simply download the packet (the link is below), enter your responses, print it out, and bring the entire packet with you to your first visit. If you happen to forget your packet, we have copies available on a table in our waiting room. However, reading and completing the New Patient Packet can take some time; it is much better to work through it before coming to RICBT. These documents are saved in Rich Text Format, so that most word processors will be able to edit them. Please note that they are not automatically submitted just by completing them; they need to be printed out and brought to your evaluation.
